Hair transplantation is a surgical technique in which hair follicles are taken from a part of the body called the ‘donor area’ to a bald or bald area of the body known as the ‘receiver area’. Because hair naturally grows in groups of 1 to 4 hairs, current techniques collect and transplant hair “follicular units” in their natural grouping. Each incision is planned in such a way that intact hair follicles are removed. The excised strip is approximately 1–1.5 x 15–30 cm in size. As the assistants close the resulting wound, they begin to examine individual follicular unit grafts, which are groups of naturally formed hair follicles smaller than the strip. Working with Binocular Stereo microscopes, they carefully remove excess fibrous and fatty tissue while trying not to damage the follicular cells that will be used for grafting. The latest closure method is called ‘Trichophytic closure’ and results in much finer scars in the donor area. The surgeon then uses very small microblades or fine needles to puncture the areas to retrieve the grafts, place them at a predetermined density and pattern and create a realistic hair. With the FUT/strip or FUE technique, the doctor can remove the tissue without the patient feeling anything. The numbing medication typically lasts for 3 hours and is touched up periodically throughout the day before it wears off, and patients may experience discomfort.

As described above, patients will feel nothing during the removal aspect of surgery after the numbing medication has been administered. However, a simplified hair transplant is completed in two parts:

It is taking grafts from the donor and replanting the grafts in the recipient areas (hairless and thinned areas). We know that after anesthesia, patients will not feel pain during surgery, but what about after surgery? What happens after the drug wears off? Fortunately, patients report little or no discomfort after surgery as well. After the surgery is over, the doctor will reapply the numbing medicine. This means that the patient will be numb for about 3 hours and any possible discomfort will occur. The patient now has 3 hours to return to his/her place of stay, settle in and most importantly start post-operative painkillers. If the patient takes the oral medication in accordance with the instructions and accumulates painkillers in his body before the numbing effect wears off, the transition process will be nice and he will not feel any discomfort. In some of these methods used, it is easier to determine the direction of the hair follicle, while in others it is directly related to the professionalism of the specialist. Baldness is removed by adding hair to the bald areas of the scalp from the nape or rarely from the chest. While doing this planting, great attention should be paid to the planting direction of the hair follicle in order to create a smooth and aesthetic appearance. Hair follicles should be separated in the right categories according to both their length and direction so that a correct image can be formed on the head after hair transplantation. Particular attention should be paid not to damage the hair follicles while performing this separation. Otherwise, hair follicles are not held by the skin in case of tissue damage, which reduces the success rate.

With this method, the angle of the channels to be opened can be reduced to 15-20 degrees and 45-50 grafts can be planted per cm2.

Sapphire FUE Technique

In the channel opening phase of hair transplantation; Instead of metal slit, grooving is performed with the help of real sapphire tips, which is the hardest element after diamond. Sapphire glass is the hardest glass in the world after diamond. With this method, the angle of the channels to be opened can be reduced to 10-15 degrees and 60-65 grafts can be planted per cm2.

Advantages of Sapphire FUE Technique

Thanks to the smooth and antibacterial surface of the sapphire tip, both trauma and tissue damage are minimized as there is less vibration during the grooving phase.

Due to less tissue and trauma damage, the recovery period after hair transplantation takes a shorter time.

It enables more frequent sowing per cm2.

Long Hair Transplant

In the Long Hair Technique, the roots are taken with special tips, without losing their originality, and planted as long. In this method, the hair, including the donor area, is not shaved in any way. In order for the healing process to be faster, the channels are opened with the Sapphire Technique. With this technique, planting is done on the front side and top regions of the areas that have started to dilute or have lost their density.

The person can return to his daily life 2 days after the operation. Examination is required for this technique to be performed. An average of 1500 – 2000 grafts can be transplanted in one session. This technique is applied in only a few clinics in Istanbul. It is not possible to apply to everyone when the cost is high.

DHI Pro (CHOI Pen) Technique

DHI (Direct Hair Implantation-Direct Hair Transplantation) DHI Hair Transplantation is the placement of individually collected hair follicles in the transplantation area with Choi implanter pens. In DHI Hair Transplantation, it is the direct transplantation of the grafts taken with a surgical pen, the thickness of which varies between 0.6 mm and 1 mm, to the scalp without opening any channels after they are separated and cleaned. This process, which is done in two separate times in normal hair transplantation methods, is called direct hair transplantation because it is done in one go with implanters in DHI Hair Transplantation.

What are the Differences of DHI Hair Transplant from FUE Technique?

While 5000-5500 grafts can be transplanted in one session in FUE technique, this number is between 4000-5000 in DHI technique.

With the DHI technique, there is less bleeding after hair transplantation compared to the FUE technique, and the recovery time is shorter.

The operation time in the FUE technique takes between 6-8 hours. In DHI technique, this period may be longer depending on the size of the area to be planted.
